What We Collect
When you submit the contact form on this site, we collect the name, email address, and message you provide. Coverage issue reports are covered below. We don't use cookies or analytics to track visitors, and we don't collect any information beyond what you submit yourself.
How We Use It
Contact form submissions are used only to respond to your message. They are not sold, shared with advertisers, or used for any purpose other than communicating with you.
Coverage Issue Reports
When you submit a coverage issue report, we collect the details you enter: your name, department, contact information, and a description of the problem with your health coverage. Reports are read only by the Local 4153 executive board and are used to document problems with the City's health plan and to advocate for fixes. We may share totals and summaries (for example, how many reports involved denied claims), but we won't share your name or identifying details without your permission. Please don't include diagnoses or other medical information.
Third-Party Services
Contact form and coverage report submissions are processed by Formspree, a third-party form service, which delivers your message to us by email. Formspree may retain submission data per its own privacy policy. We don't control or have access to any data Formspree stores beyond the messages sent to us.
Member Hub
The Members Hub login is a separate system hosted at scfrhub.com and is not part of this website. Any account information, credentials, or data you provide there is governed by that system's own privacy practices, not this policy.
