Report a Coverage Issue
The City has moved employee health coverage to ClaimDOC. If you run into a problem, like a provider refusing the plan, a surprise bill, or a denied claim, tell us here. Every report helps us document how the new coverage is working so we can push for fixes and bring real numbers to the table.
Open to all City of St. Cloud employees, not just Local 4153 members.
Before you start
- Don't include medical details. No diagnoses, medications, or treatments. We only need to know what happened with your coverage.
- Use a personal phone or computer, not a City device or City email.
- Reports go only to the Local 4153 executive board. We won't share your name without your permission.
